AGL Importer of Record profile set up issue

Hello, I'm trying to setup my profile with AGL and have entered all the required information and documents. When I submit the IOR I keep getting an error. It just says "An error occurred. Please try again."

Also, I uploaded SS-4 letter followed by LLC Member list doc. When the LLC member list doc was uploaded, the SS-4 started showing a duplicate entry - not sure why.

Can someone please help resolve this issue as we are blocked from sending our first AGL shipment.
